Clinical Care Coordinator – Value Based Programs (VBP) – Remote based in US Full Time

Job Duties/Responsibilities:

• Work with hospital Case Management team and Tenet VBP home office to optimize care plans for patients in VPBs like CJR or BPCI.
• Monitor discharge plan appropriateness using evidence-based criteria; assist in plan development.
• Promote hospital’s post-acute care network per CMS regulations on patient choice.
• Identify patients in episodic payment programs; deliver required Beneficiary Notice.
• Coordinate communication among acute care, post-acute care teams, and patients in VBPs.
• Evaluate next level of care, readmission risk, and interventions; communicate outcomes.
• Support discharge by resolving barriers, linking patients to community resources.
• Facilitate timely follow-up care with primary care providers post-discharge.
• Provide condition-specific, culturally sensitive patient education on self-care.
• Maintain PHI documentation; develop culturally sensitive transition of care plans.
• Attend meetings and provide clinical updates to maximize VBP opportunities.

JOB REQUIREMENTS:

• Graduate of accredited clinical program (Nursing, PT, OT, etc.).
• Active, unrestricted clinical certifications/licensures.
• 1-2 years clinical healthcare experience.
