Care Management Referral Specialist (NY, Remote) Full Time

Job Duties/Responsibilities:

• Receive and manage patient referrals, conduct initial calls, and arrange enrollment appointments.
• Serve as a point-of-contact for referral sources, ensuring seamless communication.
• Build strong relationships with patients and referral sources to convert referrals to active cases.
• Support care managers by rescheduling cancelled or missed visits.
• Track all referral activities and maintain detailed records in Care Management platforms.
• Participate in community marketing efforts to increase program awareness and patient referrals.
• Maintain a high conversion rate for referrals to active cases and improve referral processes.

Requirements:

• 1+ year of sales experience in healthcare or related field preferred.
• Ability to connect with people quickly over the phone and build rapport.
• Strong organizational skills and attention to detail.
• Familiarity with data management systems and documentation practices.
